One page per meeting. The section that matters is the commitments: what was agreed to, by when. A meeting note without commitments is a transcript, and nobody rereads transcripts.

Open commitments

Carried forward from every meeting until closed. This table is the first thing to check before the next meeting.

Before each meeting

Bring three things: what changed since last time, the specific decision needed, and the question that cannot be resolved alone. An advisor meeting spent on status is a meeting wasted.
